Payment and Benefits

The cost of the LIFE program depends on your Medicare and Medicaid eligibility. For participants with both Medicare and Medicaid, services are covered at 100%. For participants who are not Medicaid-eligible, there is a fee. Participants may also pay privately for LIFE services. All healthcare services, including primary care and specialist services, must be received at the Empath LIFE facility or an entity authorized by Empath LIFE. Participants may be fully and personally liable for costs of unauthorized services.
